New Patient Registration

Thank you for choosing to register with Upminster Medical Centre.

In order to register you need to live within a certain distance of our practice – please call us if you wish to confirm that you live within our ‘practice area’. If you live outside this area, your registration may be rejected (This does not apply to UAL students)

Register Online

1) New Patient Registration Form

You will also need to complete the following:

2) New Patient Health Questionnaire;

3) Audit C Template - Alcohol Screening; and

4) Summary Care Record (SCR) consent form.

Please note that the above forms should be completed for every person wanting to register with our Practice as a patient.

In addition to the above please also provide the following documents:

Photo ID: e.g. photocopy of driving licence, passport, birth certificate, current visa or identity card.

Proof of address: e.g. photocopy of council tax bill, utility bill, bank statement (dated within the last 3 months)

Mobile phone invoices are not acceptable.
